Chronic low-grade inflammation, insulin resistance, mitochondrial dysfunction, and disrupted myokine-adipokine crosstalk contribute to disease progression. Progressive resistance training combined with protein intake has the strongest clinical support. Caloric restriction without sufficient protein and muscle-preserving exercise may increase lean-mass loss.
